Kindergarten Registration will begin in March. All parents can
register at their child’s neighborhood school. If you have entered the
school lottery or have submitted a transfer request, we recommend you
still register at your neighborhood school. It is much easier to
transfer student records then identify students who have not been
registered anywhere. If you are unsure of your neighborhood school,
please contact the office of student assignment at 560-2059 or visit
the
Student Assignment Maps.

When you register your child you will need to bring these items
with you to your child's school:
- Your child’s birth certificate. This document will prove that your
child is old enough to be in Kindergarten. Children must reach 5 on or
before August 31st of their Kindergarten year to be eligible to
attend.
- Proof of Residence. This document shows you are living in the
attendance zone for your school. You can bring any one of the following
items:
a. Lease Agreement
b. Mortgage Statement or Deed
c. Durham County Property Tax Statement (home or vehicle)
d. Guardianship Documents (if necessary)
e. If the residence is not in your name, you will need to bring a
notarized statement from the Durham County resident you are living with
along with a copy of their proof of residence.
- Immunization Records. All records must have the following
information:
a. Five DPT doses (diphtheria, tetanus, and pertussis). If the forth
dose (booster) is given on or after the fourth birthday, the fifth dose
is not required.
b. Four oral polio vaccines (OPV), one of which must be a booster
given on or after the fourth birthday. If the third dose is given on or
after the fourth birthday, the fourth dose in not required.
c. Two MMR (measles, mumps, rubella) doses with the first dose on
or after the first birthday and a second dose given before entering
school.
d. At least one dose of Haemophilus influenza, b (HbOC or PRP-OMP),
given on or after the first birthday and before five years of age.
(Second dose is not required if the first dose was given after 15
months of age.)
e. Three Hepatitis B doses (not required if born before July
1994)
f. One Varicella dose on or after age 12 months and before age 19
months.
- Kindergarten Health Assessment (KHA) Form. This form is available
at your school and can be found in many doctor’s offices.
This form MUST be completed within 30 days of enrollment. Doctor's
appointments often fill up quickly before school begins, so try to make
